providers are independent contractors, not employees
of the plan or the Signature Dental Network, and
are solely responsible for the provision and quality
of professional services.

Hospital benefit
is not available in Maryland.

Waiting period refers to the
amount of time between the start of your benefits (i.e.
your effective date) and the availability of the benefits
for use. It does not refer to the length of time between
the time you enroll and the time you are active (i.e.
effective date).

Colorado and Maryland
residents are subject to a minimum $1.00 fee per service
and Washington State residents are subject to a minimum
$10.00 fee per service.
